Important: Your 2025 Health Insurance Tax Form (1094-C/1095-C)

Dear Employee/Retiree,

In accordance with the Affordable Care Act (ACA), the 2025 Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) is now available for the 2025 tax year.

While this form is not required to file your income tax return, it provides important information regarding the health insurance coverage offered to you by the School district of University City.

How to Request Your Copy: To receive a copy of your Form 1095-C, please contact the benefits department via email:

Once a request is received, the form will be furnished to you within 30 days.

In response to the federal Transparency in Coverage Rule, click here to access www.anthem.com/machine-readable-file/search (effective July 1, 2022) for machine-readable files, based on your Employer Identification Number (EIN), that include negotiated service rates and out-of-network allowed amounts between health plans and healthcare providers. These files are formatted to allow researchers, regulators and application developers to more easily access and analyze data.
